1. The Coravita Health platform
Coravita Health provides the telehealth platform and patient support. Coravita Health does not practice medicine; all diagnosis, prescribing and treatment decisions are made by independent, licensed clinicians. Prescriptions are dispensed by state-licensed U.S. pharmacies.
Coravita Health offers a telehealth platform through which independent licensed clinicians evaluate patients for medical weight management and type 2 diabetes care and, when clinically appropriate, prescribe FDA-approved medications that are dispensed by a state-licensed U.S. pharmacy. Services are available only in states where clinicians on the platform are licensed.
2. No clinician-patient relationship until accepted
Submitting a website form or starting an assessment does not create a doctor-patient or clinician-patient relationship. That relationship begins only when a licensed clinician reviews your information and accepts you as a patient.
3. Prescriptions are at the clinician's discretion
Your clinician decides, using independent medical judgment, whether any medication is clinically appropriate. We do not promise that you will receive a prescription, a particular medication, or a particular dose. Medications are prescribed only for their FDA-approved uses.
4. Not for emergencies
Our services are not for medical emergencies. If you think you are having a medical emergency, call 911 or go to the nearest emergency room.

Coravita Health is not affiliated with or endorsed by these manufacturers. All product names are used only to identify the FDA-approved products a clinician may prescribe.
11. Website content and acceptable use
The website and its content are owned by CORAVITA HEALTH LLC and protected by law. You agree not to misuse the website, interfere with its operation, or submit information that is false or belongs to someone else.
12. Limitation of liability
To the fullest extent permitted by law, CORAVITA HEALTH LLC is not liable for indirect, incidental, or consequential damages arising from your use of the website. Nothing in these Terms limits liability that cannot be limited under applicable law, including for professional medical services provided by independent, licensed clinicians.
13. Governing law
CORAVITA HEALTH LLC is a Delaware limited liability company. These Terms are governed by the laws of the State of Delaware, without regard to conflict-of-law rules. Any dispute will be resolved in the state or federal courts located in the State of Delaware.
