This Privacy Policy explains how CORAVITA HEALTH LLC, doing business as Coravita Health ("Coravita Health," "we," "us," or "our"), collects, uses, shares, and protects information when you visit coravitahealthcare.com, contact us, receive text messages from us, or use our telehealth services.
Information we collect
Information you give us on this website
Our website contact forms ask only for your name, email address, phone number (optional), state of residence, and an optional short message. Please do not include medical details in these forms.
Health information collected through our secure intake
If you start an assessment, health information, such as your medical history, current medications, height and weight, and lab results, is collected through a secure intake so that an independent, licensed clinician can evaluate you for care.
Information collected automatically
When you visit our website we may collect standard technical information, such as browser type, device type, pages visited, and approximate location derived from your IP address.
How we use information
- To respond to your inquiries and send you a link to our secure assessment.
- To operate the telehealth platform through which independent, licensed clinicians provide evaluation, prescribing when clinically appropriate, and follow-up care. Coravita Health does not practice medicine.
- To coordinate dispensing with a state-licensed U.S. pharmacy and to help check whether your pharmacy benefit covers a prescribed medication.
- To send account notifications and, if you opt in, marketing text messages as described below.
- To operate, secure, and improve our website and services, and to comply with law.
Health information and HIPAA-aligned safeguards
Your health information is used only to deliver your care. We never sell it.
We handle health information under safeguards aligned with the Health Insurance Portability and Accountability Act (HIPAA), including administrative, physical, and technical protections. Where HIPAA applies to the clinicians or pharmacy involved in your care, their Notice of Privacy Practices also governs your information.
Health information is shared only with your treating clinician and the dispensing pharmacy to deliver your care, and with service providers who support that care under written confidentiality obligations (including business associate agreements where required). We do not use health information for advertising.
How we share information
- Your care team: your treating clinician and the dispensing state-licensed U.S. pharmacy.
- Service providers: companies that host our systems, process communications, or provide technical support, bound by contracts that limit their use of your information.
- Legal requirements: when required by law, subpoena, or to protect the safety of you or others.
- Business transfers: in connection with a merger or sale of assets, subject to this policy.
SMS Consent and Terms
By providing your phone number and checking the SMS consent box, you consent to receive recurring SMS communications from Coravita Health (CORAVITA HEALTH LLC).
We offer two types of text messages, each with its own consent: account notifications (appointment reminders, visit status updates, intake reminders, prescription status and refill reminders, shipping updates, and two-factor authentication codes) and marketing messages (program information, educational health content, and visit availability). Marketing text messages never name a prescription medication.
SMS Terms
- Message frequency varies.
- Message and data rates may apply.
- Reply STOP to opt out at any time. You will receive one confirmation message and no further messages of that type.
- Reply HELP for assistance, or contact us at (561) 400-2323 or [email protected].
- Carriers are not liable for delayed or undelivered messages.
We Do Not Sell Your Data
We do not sell, rent, or share your personal information with third parties for their own marketing purposes.
Third-Party Sharing
Your SMS opt-in information and consent will not be shared with any third party for marketing purposes. Mobile information is shared only with service providers that deliver our messages.
Cookie and Tracking Disclosure
Our website may use cookies and similar technologies to improve your experience and understand site usage. You can disable cookies in your browser settings. We do not place advertising trackers on pages where you enter health information.
Your consent to receive SMS is not required as a condition of purchase or of receiving care.
Data security and retention
We use reasonable administrative, technical, and physical safeguards to protect information. No method of transmission or storage is completely secure. We keep information only as long as needed for the purposes above, including medical-record retention periods required by law.
Your choices
- Opt out of text messages at any time by replying STOP.
- Opt out of marketing emails using the unsubscribe link in any email.
- Request access to, correction of, or deletion of your information by emailing [email protected], subject to medical-record retention requirements.
California Privacy Rights (CCPA)
California residents have the right to know what personal information we collect, use, and disclose; to request deletion; to correct inaccurate information; to limit the use of sensitive personal information; and to opt out of any sale or sharing (we do not sell your data or share it for cross-context behavioral advertising). We will not discriminate against you for exercising these rights. Medical information governed by HIPAA or California's Confidentiality of Medical Information Act is handled under those laws. Contact [email protected] to exercise these rights.
Other U.S. state privacy rights
Depending on where you live, you may have additional rights under state privacy and consumer health data laws, such as the right to confirm whether we process your personal data, to access, correct, or delete it, to obtain a copy, and to opt out of targeted advertising, sale, or profiling. We honor these requests regardless of your state. To exercise a right, call (561) 400-2323 or email [email protected]. If we deny your request, you may appeal by replying to our response.
